INSURANCE INFORMATION

Physical Therapy is covered by most major insurance plans. 
In all cases for both in and out-of-network, Gotham Physical Therapy will research your coverage and benefits and inform you of any limitations prior to your visits.

IN-NETWORK

Gotham Physical Therapy is in-network with the following Insurance Companies:

Medicare

Aetna – all plans

Oxford – all plans

We will file for No-Fault and Workers Compensation providing the case is current and the patient has all the necessary information.

 OUT OF NETWORK

If your insurance company is not one of the above, we will contact them to find out if our services can be provided to you out-of-network.  This office for your convenience will submit the billing.  Below is a list of out-of-network insurance companies that we have worked with:
